COVID-related use case: How about an app which allows me to share my current health data (body temperature) + name + email at the time of checking into a building/site/public space/airport?

See OpenHumans.org They are working in a way on something like this. But to much still for the technician and not setup for normal users.

We as cluster headache community do use them so the data of our individual members is stored in their ownership.
Aside our own community app we can collect Fitbit, DNA, GPS and other data sources and it growing
